Position Title – Population Health Project Assistant

Position Summary – Part time up to 24 hours per week

Reports to -  Director of Business Development and Measures Management

Supervises - N/A

Tasks List:

Position will assist directors or managers who are also team leads for Accountable Care Organizations

(ACOs) and the Delivery System Reform Incentive Payment (DSRIP) Program. Primary responsibility will

include scheduling and coordinating appointments via telephone between NJII's nurse abstractors and

offices of participating providers. It will also include addressing email requests for appointments related

to ACO and/or DSRIP abstraction, and providing general assistance to callers related to ACO and DSRIP.

The position will also include assisting in daily operational activities at NJII, creating and submitting

expense and travel reports for the nurse abstractors, working directly with NJII clients, healthcare

providers, hospital teams, etc. The tasks assigned to this position could vary daily/weekly. Work

schedule 24 hours per week 8:30-4:30 pm on Monday thru Friday with flexibility for extra coverage

when needed.

Additional duties -

        Assists in managing project development, liaising with project stakeholders on an

           ongoing basis

        Identifies and resolves issues and conflicts within the project team

        Develops and delivers progress reports, requirements, documentation, and presentation

Qualifications -

       1. Education requirements: Bachelor’s or Associate’s degree required

       2. Experience requirements: Prior experience in healthcare industry required

       3. Knowledge/Skills requirements: 

                     Must possess excellent customer service and communication skills

                     Must be comfortable using computer and telephone

                     Well-organize with a positive and upbeat attitude

                     Population health experience preferred.

                     Knowledge of Microsoft Office Suite and Workflow principles
